Mperfect Wellness is honored to have been selected by the Home Centered Care Institute (HCCI) to participate in the Illinois House Call Project, a statewide initiative dedicated to expanding access to high-quality home-based primary care (HBPC) for homebound patients. With the support and guidance of HCCI, participating practices embark in a transformational journey to improve patient outcomes, increase awareness of HBPC, and contribute to the goal of serving 6,000 new home-based primary care patients across Illinois by the end of 2026.
